  • We offer a sliding fee discount for those without insurance. Discount is based on household size and income

Need to talk to a provider but our office is closed?

You can reach one of our providers 24/7 by calling the after-hours line at 419-238-2390. Staff at Van Wert Health will transfer your call to our on-call health care provider to assist you with your health care needs. If you need a medication refill, please request it on our online portal or call us during regular business hours.
