History
Family Health Care began as the "Family Caring Clinic" in 2004 with one nurse practitioner and two exam rooms in the same building as a local home health agency.
Before this, people without health insurance or who were under-insured had no where to go for health care other than the local emergency department.
In 2007, we were recognized as a Federally-Qualified Health Center Look-alike which allowed us to offer more services. In 2008, we hired our first medical doctor –Dr. Tom Lautzenheiser. We continued to grow over the next seven years as we added services and moved to bigger buildings.
2015 was a big year for us as we became a full Federally-Qualified Health Center, which allowed us to rapidly expand our services and move into our current location. In 2017 we added dental and behavioral health services. We added care management services in 2018 and clinical pharmacy services in 2019.
From our humble beginnings to where we are today, we have been dedicated to providing health care for everyone, regardless of their ability to afford health care.
Leadership
We are fortunate to have a dedicated Board of Directors who are committed to our mission and over half of them are also patients of our community health center.
Our CEO, Jennifer Smith, has been with Family Health Care since 2005 and became the CEO in 2013. She has been part of the organization and seen it grow from a one provider clinic, to an FQHC-look alike, to a fully funded 330 grantee Federally Qualified Health Center. Jennifer is honored to be part of an organization that is mission driven and fulfilling a need within the community. She received her bachelor’s degree in Business Administration from the University of Northwestern Ohio. Jennifer lives in Van Wert and is actively involved in the community with her husband and their 3 children.

The quality of our health care services has to keep getting better every year in order for us to keep our awards and recognition -- a challenge we happily accept.

Family Health Care is a Federally-Qualified Health Center (FQHC) committed to: charging fees based on the patient's ability to pay, providing quality health care to all regardless of insurance status or income level, staffed by highly competent health care professional teams, community controlled by a patient majority board of directors.
